*Doctor’s Note Requirement
To qualify an absence as excused for illness, injury, or medical appointment, the following criteria must be strictly met:
A written note from a licensed healthcare provider must include:
- Provider’s signature
- Student’s name
- Date(s) of excused absence
- Medical reason for absence
- Date student may return to school
Notes lacking any of the above elements including provider signature and duration will not be accepted. Absent such documentation, the absence may be classified as unexcused.
For absences of 3 consecutive days or more due to illness or injury, a healthcare provider note is mandatory to qualify as excused.
Documentation must be submitted within two (2) school days of the student’s return following an absence. Submission via written note or electronic upload is acceptable.
*School Accident Insurance Policy
The school maintains a Student Accident Insurance Policy covering injuries sustained during school-sponsored activities.
This policy is supplemental to family personal health insurance; it does not replace it.
Parents/guardians are hereby informed of the policy’s existence and may request full details including coverage limitations, claim procedures, by contacting the school.
